Individual Plan of Service (IPOS)

An Individualized Plan of Service (an IPOS or Plan) is developed from your person-centered planning process that begins with pre-planning. The IPOS is referred to as a Plan.

The IPOS/Plan is for you. It is about you.

In your IPOS/Plan, developed through the person-centered planning process, there are:

The goals and outcomes identified in your person-centered planning meeting.

Any services or support that you will need to achieve the outcomes you need to achieve your personal recovery goals.

Your IPOS/PLAN should support you in achieving your personal recovery goals which includes:

Getting your needs met

Connecting with people in your community

Taking on a valued role in your community.

When the services and support you need are placed in your Plan, those needs must be medically necessary. This means that the outcomes in your Plan are meant to:

Treat or stabilize your symptoms

Stop symptoms from getting worse, or

They are designed to assist or maintain a high enough level of wellness that you achieve your goals of community inclusion, participation, independence, recovery or productivity.

In your IPOS are tools, answers, support and services to help you get to your personal recovery goals.

Your plan will state:

The number of times the service will be provided (amount)

How long it will be provided for (duration) and

The way the service will be provided (scope)

Who will provide it

How it will happen

Where it will happen

Everything in your plan needs to be clear so you know what to expect and how things will happen. When you know what to expect and how things will happen, this means that you are an informed consumer. You know what is going to happen and when.

Your mental health provider approves your IPOS/Plan.

You are to get a copy in 15 days.

It's your right to have a copy in 15 days.

When you get the copy, take the time to look it over again; make sure it is what you expect, want and need. Make sure the services that are being provided are what you wanted when you and your circle of support helped design your Plan.
