Integrated Health

The Michigan Department of Community Health recognizes the need to address the “whole health” of people living with mental illness. This commitment is grounded in the sobering knowledge that persons with serious mental illness die on average 25 years earlier than the general population. This section provides information about integrated health and wellness initiatives taking place in Michigan as well as tools for effective collaboration among members of the Michigan Integrated Health Learning Community.

 

On May 18, 2010 mental health leaders from across the state presented "Achieving Integration of Physical and Behavioral Health Care" at the Michigan Association of Community Mental Health Boards 2010 Spring Conference.

Presenters included:

Michigan Integrated Health and Wellness - Join the list serve

The Michigan Integrated Health Learning Community is an initiative funded by the Michigan Department of Community Health and The Michigan Primary Care Association that links Federally Qualified Health Centers, Community Health Centers, Community Mental Health Centers and Prepaid Inpatient Health Providers together through a variety of resources including this website, regular meetings and coaching provided by consumers and staff trained in IH implementation.

"Integrated Health Care programming" means that physical and mental health, including substance abuse services, are provided in one location or with strong collaboration between two or more separate organizations by providing welcoming and easy to navigate servcies.  Providers work together as a single team for the benefit of the consumer and the focus in on collaboration, wellness and recovery.
